Group healthcare insurers must provide a certification of coverage:
• At the time an individual ceases to be covered under a healthcare insurance plan or becomes covered under federal COBRA;
• At the time an individual ceases to be covered under federal COBRA; and
• Upon request by an individual or on behalf of an individual within 24 months after the date coverage under the healthcare insurance plan or federal COBRA ceases (AK Stat. Sec. 21.54.120).
