Hawaii has adopted the federal workplace illness and injury recordkeeping and employee medical record requirements by reference (HAR 12-50-9 for recordkeeping and HAR 12-202-3.1 for medical records).

Reporting fatalities and multiple hospitalizations. Employers must report any accident resulting in one or more fatalities or the hospitalization of three or more employees to the nearest regulatory agency office within eight hours of either the occurrence or the time it became known to any agent of the employer by calling 808-586-9102.
