Employers must report any accident resulting in one or more fatalities or the hospitalization of three or more employees to the nearest office within 8 hours of either the occurrence or the time it became known to any agent of the employer. If during normal business hours (8:00 a.m. to 4:30 p.m., Monday to Friday), call 317-232-2693. Identify the call as a fatality or catastrophe report. If not during normal business hours or on a holiday, call the federal OSHA Hotline at 1-800-321-6742.
