Iowa has adopted by reference the federal requirements for illness and injury recordkeeping and employee medical records in general industry workplaces (IAC 875-4.1 to 875-4.3 and IAC 875-10.3).
Report of fatality or hospitalization. The state has adopted the federal rules that require employers to report fatalities or the hospitalization of three or more employees to the nearest state OSHA office within 8 hours of either the occurrence or the time it became known to any agent of the employer. Iowa employers can call 877-242-6742 to make these reports 24 hours a day, including weekends and holidays.
The Iowa Workforce Development/Division of Labor Services administers and enforces workplace safety rules for private and public sector workplaces in the state.
