Accident report hotline. All employers in Washington covered by workplace safety and health rules must report a fatality, probable death, or hospitalization incident that involves any employee within 8 hours of the incident by calling L&I, or the federal Occupational Safety and Health Administration (OSHA) at 800-321-6742.
