Air – Start-up, shutdown, and malfunctions,10 CSR 10-6.050(4) Facilities must maintain the following information for any malfunction that results in excess emissions for more than 1 hour: • Name and location of installation; • Name and telephone number of person responsible for the installation; • Name of the person who first discovered the malfunction and precise time and date that the malfunction was discovered; • Identity of the equipment causing the excess emissions; • Time and duration of the period of excess emissions; • Cause of the excess emissions; • Air pollutants involved; • Estimate of the magnitude of the excess emissions expressed in the appropriate units and calculations used in estimating the magnitude; • Measures taken to mitigate the extent and duration of the excess emissions; and • Measures taken to remedy the situation that caused the excess emissions and the measures taken or planned to prevent the recurrence of these situations.
